Left crankarm fall off

[attachment=910][attachment=911][attachment=909][attachment=909]My left crankarm keeps falling off after a couple of hours after riding my bike. I don’t know if it’s just because it’s not on far enough or if the piece is rounded out.

G_M is correct about the photos not displaying. What type of BB spindle/crank fit (taper, splined, etc.)?
